Transforaminal Endoscopic Surgery: Outside-In Technique

Abstract: Transforaminal endoscopic lumbar discectomy (TELD) with the outside-in technique can be applied to nearly all cases of lumbar disc herniation (LDH), and transpedicular endoscopic lumbar discectomy can be used to treat highly migrated LDHs. The purpose of this study was to outline these 2 outside-in surgical techniques and to present their clinical outcomes. Between January 2018 and January 2019, a total of 137 patients underwent either transforaminal or transpedicular endoscopic lumbar discectomy. We performed… Show more

“…Alternative foraminoplasty techniques are (1) drilling the ventral aspect of the superior facet using a high-speed drill (Joimax® Shrill, 3.5 mm diamond burr) under direct endoscopic visualization and (2) using a Jamshidi needle to pass through the superior articular facet to reach at the midpedicular level on AP fluoroscopic view. A guidewire was inserted through the needle followed by serial dilation with side cutting bone drill inserted over the wire [ 3 , 8 , 14 , 15 ] ( Figure 3 ).…”

“…Full endoscopic spine surgery is performed through a working-channel endoscope. The basic concept for the transforaminal or interlaminar approach is similar to the lumbar spine [ 8 10 ]. Endoscopic approaches to the thoracic spine have been reported and found to be safe and effective as well as avoiding approach-related complications with injury to the visceral and vascular structure [ 1 , 2 , 11 13 ].…”

“…112 Transforaminal endoscope is introduced through serial dilation. 58,111 A mobile outside in technique with endoscopic drilling of ventral superior articular facet was performed to gain access to the intervertebral disc space 47 . Parts of disc were typically removed to accommodate the introduction of transforaminal endoscope to sinuvertebral nerve and basivertebral nerve.…”

“…The proposal of this concept laid a theoretical foundation for the extensive development of lateral spinal endoscopy. Since then, Yeung from the United States and Hoogland from Germany proposed Yeung endoscopic spine system and transforminal endoscopic spine system technologies, respectively, which served as the prelude to the global popularity of spinal endoscopy [5][6][7]. However, people remain skeptical about the new technology represented by PTELD due to concerns about a lack of high-level evidence, follow-up, and replicability.…”
