Transformed non‐Hodgkin lymphoma in the rituximab era: analysis of the NCCN outcomes database

Abstract: SummaryHistological transformation (HT) is a major cause of morbidity and mortality in patients with indolent non-Hodgkin lymphoma (NHL). The multicentre National Cancer Comprehensive Network database for NHL provides a unique opportunity to investigate the natural history of HT in the rituximab era. 118 patients with biopsy-confirmed indolent lymphoma and subsequent biopsy-confirmed HT were identified. Treatments for HT included autologous stem-cell transplant (auto-SCT) (n = 50), allogeneic SCT (allo-SCT) (n… Show more

“…In this multi-institutional, prospective, observational study, the 5-and 8-year transformation rates were 12.8% and 19.4%, respectively, which is consistent with the incidence of 9% to 17% at 5 years and 15% to 30% at 10 years observed in retrospective studies. [3][4][5][6][7]9 The observation in other series, 10,15 as well as our own, that transformation rates are similar in the modern era suggests that †Patients with at least a response of stable disease to an initial rituximab-based regimen who were followed at least 215 days after the end of induction without experiencing progressive disease or starting a subsequent therapy within this time frame were included in analysis. chemoimmunotherapy is not impacting the incidence of transformation.…”

“…The median posttransformation OS of nearly 5 years in the NLCS is similar to recent series of tFL. 10,15 Compared with the dismal prognosis described in historical reports, these newer studies raise the question as to whether this change reflects advancements in current treatments. The disparity seen in the relatively short median posttransformation PFS of ;1 year compared with the longer OS additionally suggests improvements in treatment.…”

“…Transformation into DLBCL has been reported at a rate of 3% per year in patients with follicular lymphoma [22], but aggressive transformation may occur in every subtype of indolent NHL [23], including WM [24,25]. Patients who develop histologic transformation experience poor survival [22], which may have improved with the addition of rituximab to chemotherapy and highdose chemotherapy followed by autologous stem cell rescue [23].…”

“…30 In several studies of transformed lymphoma, ASCT consolidation appears to provide benefit even in the rituximab era. [56][57][58] The benefit of ASCT on patients with double-hit transformation has not been established in these studies, but no doubt a subset of patients included in these retrospective analyses of transformed lymphomas had double-hit lymphoma at the time of transformation.…”
