Transfusion‐related acute lung injury (TRALI) following autologous stem cell transplant for relapsed acute myeloid leukaemia: a case report and review of the literature

Abstract: A fatal case of transfusion-related acute lung injury (TRALI) in a child post-autologous stem cell transplant for relapsed acute myeloid leukaemia is described. The implicated product was a single unit platelet concentrate containing anti-HLA A2 and granulocyte-specific anti-NA1 antibodies. The recipient typed as HLA A2/A2, NA1/NA1. This is the first reported case of TRALI following a transplant procedure for a haematological condition. It is also unusual in that the patient failed to make a full recovery and … Show more

“…Serological work up of TRALI patients identified antibodies to a HNA in a number of cases (Yomtovian et al, 1984;Nordhagen et al, 1986;Bux et al, 1996;Leach et al, 1998); a description of HNA antigens and their implication in TRALI has been published previously (Bux, 2005).…”

“…6,17,20,39,41,42 TRALI has also been reported in patients requiring transplantation of bone marrow and solid organ; ALI in these patients should be considered an untoward reaction related to the graft or to supportive care. [43][44][45][46][47][48][49][50] Further prospective analyses of patients who have undergone transfusion are required to properly identify patients who are at risk for TRALI.…”

“…TRALI has been reported following transfusion of plasma-containing blood components [13,[17][18][19][20][21][22][23][24][25]. Estimates of the incidence of TRALI include: 1 in 5000 blood and blood components transfused [17]; 1 in 2000 plasma-containing components [26]; 1 in 7900 units of fresh frozen plasma [27]; 1 in 432 units of whole bloodderived platelet concentrates [18]; and 1 in 1323 blood components transfused [18].…”
