Transient Monoclonal Gammopathy Associated With Cytomegalovirus Infection

Abstract: A transient monoclonal gammopathy, IgA-lambda light chain, associated with a cytomegalovirus (CMV) infection occurred in a patient with acute leukemia in remission. Evidence of CMV infection was confirmed by indirect hemagglutination titers, initially being 1:4096 and gradually falling to 1:64. During the infection, serum and salivary IgA levels rose to 2450 mg/100 ml and 72 mg/100 ml, respectively. Bence Jones proteinuria, lambda type, (0.1 g/24 hr) occurred during the period of excessive immunoglobulin synth… Show more

“…CMV infection has been evidenced in two of four control‐transplanted patients with gammopathy and in zero of seven control patients without gammopathy. CMV infection testing has been limited in our retrospective study, but our observation supports that of others where CMV infection has been associated with transient gammopathy appearance in transplanted patients (38, 39, 43–46). Indeed, CMV infection activates B cells and depresses cell‐mediated immunity (47, 48).…”

“…Of interest, we identified development of a monoclonal dysproteinemia occurring concurrently with PyVAN in two patients. A transient monoclonal gammopathy has been reported previously in a patient with CMV infection . The clinical significance of our findings cannot be ascertained at this time, but highlights the prominent role of plasma cells in PyVAN, presumably related to this phenomenon.…”

“…Transient paraproteinemia has been reported in the setting of various infectious and autoimmune diseases 70 , 71 , 72 . While not previously described, it is possible that SARS-CoV-2 can induce transient paraproteinemia due to hyperactivity of the immune system in response to infection.…”

“…While not previously described, it is possible that SARS-CoV-2 can induce transient paraproteinemia due to hyperactivity of the immune system in response to infection. Transient paraproteinemia has also been associated with kidney dysfunction 70 . It is unknown whether this accounts for the increased frequency of proliferative glomerulonephritis with monoclonal IgG deposits in COVID-19 patients.…”
