Transient Part-Solid Nodules Detected at Screening Thin-Section CT for Lung Cancer: Comparison with Persistent Part-Solid Nodules

Abstract: A substantial proportion of PSNs detected at screening CT were transient. Transient PSNs could be predicted with high accuracy by using the features of young patient age, detection of the PSN at follow-up, blood eosinophilia, lesion multiplicity, large solid portion, and ill-defined lesion border.

“…The high rate of transience represents the prevalence of eosinophilic pulmonary infiltrates in Korea, as about 33% of transient nodules were eosinophilic pulmonary infiltrates in our series. This trend is not restricted to a region, as other reports 8,10 from hospitals in a metropolitan capital city also reported the high prevalence of transient nodules in association with eosinophilia. We believe it is related to a dietary habit of Korean people.…”

“…Recently, Lee et al 10 reported a 69.8% of transience rate from 126 partly solid GGO nodules. They also observed that young patient age, blood eosinophilia, lesion multiplicity, large solid portion and ill-defined border were significant independent predictors of transience.…”

“…Transient SSNs are associated with infectious or inflammatory conditions, and blood eosinophilia is one of the wellknown causes of transient inflammation in the lung (1, 37). Lee et al (37) reported that transient SSNs are related to features such as a young patient age, detection at follow up, blood eosinophilia, lesion multiplicity, a large solid portion, and an ill-defined lesion border.…”

“…A single PSN is considered malignant until proven otherwise if it remains stable or grows on a 3-month follow-up scan An initial 3-month follow-up CT is recommended with PSNs to ensure persistence [16] . Malignant lesions may occasionally show a temporary decrease in size, due to contraction of a fibrotic or atelectatic component, and therefore a slight decrease in size should not constitute evidence of benignancy [17] .…”
