Transition: a literature review

Abstract: Transition is the way people respond to change over time. People undergo transition when they need to adapt to new situations or circumstances in order to incorporate the change event into their lives. Transition is a concept that is important to nursing; however, to further develop understandings, research must extend beyond single events or single responses. Longitudinal comparative and longitudinal cross-sectional inquiries are required to further develop the concept.

“…20 Early in the analysis, it became clear that participants' narratives about the future were strongly shaped by and described in terms of past experiences, particularly past transitions. This prompted us to subsequently use transition theory [22][23][24] as a guiding framework to help focus our analysis. According to transition theory, people's responses and adaptation during change such as serious illness can result in new ways of how they view themselves.…”

“…11 It is thus a complex process that calls for constant adjustment on several dimensions. 12,13 One definition of transition often used in the literature is that proposed by the Society for Adolescent Medicine, 14 which regards transitioning as a deliberate, planned process that addresses the medical, psychosocial, vocational, and educational needs of adolescents and young adults with chronic conditions when moving from a pediatric service to adult-oriented care. This transition should be recognized as merely one part of the broader set of educational, personal, family-related, and social transitions that adolescents experience.…”

“…Hence, in the health care context, the word "transition" carries the additional meaning of a process of psychological adaptation to a change or break. 13 …”
