Transitions from neonatal intensive care unit to ambulatory care: description and evaluation of the proactive risk assessment process

Abstract: While HFMEA holds promise for improving the safety of care transitions, the full effort required to realise the potential benefit requires additional evaluation to confirm its value over less intensive means of achieving safer care transitions.

“…A previous study investigated the prevalence and potential risk of medication handling errors, corresponding knowledge deficits in nurses and parents committing the errors [23]. In over 40% (among nurses) and 96% (among parents) of all observed processes medication handling and administration errors were detected [23]. We postulate that this proportion would be higher in the migrant population, due to language barriers, and different perception and cultural background may add to the percentage.…”

“…Systematic strategies for their prevention, however, are still lacking. “…11 This is a reflection of the fact that severe events rarely occur and their detection is considered high given the controls and checks that already are in place. 16 Furthermore, a recent study found considerable variability in hazard scoring results when comparing individual scoring with a team consensus procedure, which rather confirms the subjectivity of this method. 16 Furthermore, a recent study found considerable variability in hazard scoring results when comparing individual scoring with a team consensus procedure, which rather confirms the subjectivity of this method.…”

“…22 There are several limitations to this study. 15,16,20,21,24 Fourth, the lack of quantitative outcome data, which is inherent to the method, increases the difficulty of comparing the benefits of the different HFMEA variants. Second, the teams were not fully multidisciplinary composed because there was no representation of radiation oncologists or clinical physicists.…”
