Transjugular Intrahepatic Portosystemic Shunt versus Endoscopic Sclerotherapy in the Elective Treatment of Recurrent Variceal Bleeding

Abstract: The present study was designed to compare elective transjugular intrahepatic portosystemic shunts (TIPS) and endoscopic sclerotherapy (EST) in terms of their efficacy in preventing recurrent bleeding from gastro-oesophageal varices in patients with advanced liver cirrhosis and portal hypertension. Of 96 patients with at least three gastro-oesophageal variceal rebleeds, 50 were treated with elective TIPS and 46 with EST. Recurrent variceal bleeding was significantly more frequent in patients receiving EST treat… Show more

“…From an initial 395 studies, 214 records were screened and 33 full-length articles were assessed. Twenty-one studies were included in the final analysis, of which 12 studies reported on the outcomes with SEMS [7-9, 26-34] and 9 reported on the outcomes with TIPS [6,[35][36][37][38][39][40][41][42].…”

“…TIPS was placed using a technique described in available literature, the procedure took place in the interventional radiology suite. 12 , 15 Prior to elective TIPS, patients were hemodynamically and systemically stable. After indirect portography between the portal and hepatic veins, shunt tracts were lined with wallstent endoprosthesis (Wallstent, Schneider, Switzerland) or polytetrafluoroethylene-covered stents (GoreÒViatorrÒ; United States).…”
