Translation of Obesity Practice Guidelines: Measurement and Evaluation

Abstract: System-level PHN practice facilitator interventions successfully translated clinical obesity guidelines into interprofessional use in health care organizations. The Omaha System Problem Rating Scale for Outcomes reliably measured system-level outcomes.
