Transmural Dispersion of Repolarization in Failing and Nonfailing Human Ventricle

Abstract: Rationale: Transmural dispersion of repolarization has been shown to play a role in the genesis of ventricular tachycardia and fibrillation in different animal models of heart failure (HF). Heterogeneous changes of repolarization within the midmyocardial population of ventricular cells have been considered an important contributor to the HF phenotype. However, there is limited electrophysiological data from the human heart. Objective: To study electrophysiological remodeling of transmural repolarization in the… Show more

“…In normal myocardium, longitudinal and transverse intracellular conductivity values were scaled to 0.255 and 0.0775 S/m, respectively. This resulted in longitudinal and transverse conduction velocities (CV L = 59.9 and CV T = 33.5 cm/s, respectively) consistent with values measured in experiments with human ventricular tissue (49,50). In the PIZ, transverse intracellular conductivity was reduced by 90% to represent connexin-43 remodeling observed in the infarct border zone (51).…”

“…Nonetheless, the ORd model was developed to model normal physiological AP waveforms, and considers the controversial presence of a large number of M cells in a ventricular strand. 73 Our application extends this data set to a substrate that is a target for clinical anti-arrhythmic agents or drug candidates new in development.…”

“…This technique has been extensively used by others in wedges from explanted human hearts 17, 18, 19, 20, 21, 22. Briefly, wedges of porcine myocardium were dissected from the mid apico‐basal region adjacent to the anterior infarct, a region known to support post‐MI arrhythmias.…”
