Endoscopic thyroid surgery has gained popularity with the advances in laparoscopic and endoscopic instruments and techniques. Of all endoscopic approaches for thyroidectomy, the transoral endoscopic thyroidectomy vestibular approach (TOETVA) is the most promising procedure as it avoids neck scars and has less tissue dissection than other endoscopic techniques. This study aimed to present our first impression and initial experience with TOETVA in Erbil city. Twelve patients underwent surgery between November 2020 and April 2021, including eleven females and one male. Eleven total thyroidectomies and one lobectomy were performed. We found no significant postoperative complications. TOETVA is a safe and effective procedure, but it is challenging, needs good skills, and is a promising technique.

Outcomes of Per-oral Endoscopic Thyroidectomy (POET) – A New Trend for Scarfree Thyroidectomy

Objectives: This paper discusses the surgical outcomes of POET compared to open thyroidectomy (OT) and other approaches of video thyroidectomy (VT) such as chest or axillary approach to investigate the feasibility of POET as a safe alternative. Background: Per-Oral Endoscopic Thyroidectomy (POET) is a developing novel technique based on the principles of minimally invasive surgery offering “scar-free” thyroidectomy. To date, results from various individual centers have been published internationally but none from UK, Malaysia or Singapore. Methods: This is a single tertiary hospital, single surgeon based retrospective analysis of 98 selected patients (mean age: 43.5 years old) undergoing POET in 12 months (July 2017 to July 2018). It records the complication rates post-operatively until day of discharge. Results: The average length of stay is 2 days and average time of operation is 69 minutes. There was no blood loss >100ml and no wound infection recorded among the 98 patients. Only 1 out of 98 patients (1%) had permanent recurrent laryngeal nerve (RLN) palsy. Clinical hypocalcemia described as cramp and lip paresthesia was experienced by 6 patients (6%). 5% had transient mental nerve injury described as mental paresthesia. Conclusion: POET is a promising safe alternative technique to thyroidectomy with comparable surgical outcome yet significantly better cosmetic outcome. This technique has a strong potential to be useful when lowering the rate of developing keloidal scar and hypertrophic scaring. POET can be an option to offer patients who are especially driven to achieve perfect cosmesis.

Introduction: thyroid gland surgical excision has been performed since the 19th century, which was first employed by Theodor Kocher, this technique since its inception presented certain post-surgical complications and left an aesthetically uncomfortable scar in patients; however a few years ago new techniques have been carried out under laparoscopic approaches which leave less scarring; under this same concept, in 2016 the TOETVA technique (Endoscopic trans-oral thyroidectomy with vestibular approach) is presented, with which fewer postoperative complications have been evidenced. Objective: To identify post-surgical complications, surgical and hospitalization time between TOETVA versus endoscopic techniques. Materials and Methods: a retrospective study of two surgical interventions was carried out, which included patient’s surgery by Endoscopic Trans-oral Thyroidectomy and patient’s surgery by other endoscopic techniques at Hospital General IESS Ibarra in the period between January 2017 - March 2019. Thirty-three patients met the inclusion criteria, 19 patients were tested for Endoscopic Trans-oral Thyroidectomy with TOETVA vestibular approach, and 14 with cervical endoscopic thyroidectomy. Descriptive and inferential statistical analysis was performed using SPSS software. Version 19. Results: the entire studied population was female, with 45.3 years old in average, 42.42% resided in Ibarra, while 15.5% were from Otavalo, these two populations were the most representative. Regarding to the issue, 68.4% patient’s surgery by TOETVA technique and 71.42% % patient’s surgery by cervical endoscopic thyroidectomy technique did not present complications. The surgical time in which it was reported that Endoscopic Trans-oral Thyroidectomy with vestibular approach was also studied, TOETVA had a mean of 129.84 minutes, while the cervical endoscopic had a mean of 131.07 minutes. The hospital stay was shorter for cervical endoscopic thyroidectomy due to complications presented with TOETVA with a mean of 6.32 and 4.14 days respectively. Conclusions: trans-oral Endoscopic Thyroidectomy with vestibular approach (TOETVA) is a technique that is in the process of implementation and learning in Ecuador and that as a result of the present investigation it could be concluded that there are no significant differences regarding post-surgical complications between the techniques studied.
