Transrectal Ultrasonography for Prostatic Cancer: Its Value in Staging and Monitoring the Response to Radiotherapy and Chemotherapy

Abstract: Current methods to evaluate the size and local extent of prostatic cancer are imprecise. Quantitative assessments of changes after therapy are unreliable. We have used transrectal ultrasonography in 50 patients with clinically localized prostatic cancer to determine the value of this imaging technique in staging the local tumor and in monitoring the response of the tumor to therapy. Transverse images of the prostate were obtained at 5 mm. intervals, and were used to determine the size and shape of the prostate… Show more

“…As TRUS allows accurate images of the prostate due to a better resolution of the prostate margins, methods of prostate gland volume determina tion were established [1], Today, estimation of the pros tate gland volume by TRUS measurement, which is obtained in transverse and longitudinal scanning views, has become normal clinical practice. Although several studies have been performed, most of them employ only a single method to estimate the prostate volume [2][3][4][5]. In addition, the majority either lack controls or are based on transurethral resection weights.…”

“…It could be observed that prostatic carcinomas infil trating the capsule and beyond can be detected and staged relatively reliably by means of transrectal sono graphy [5]. However, carcinomas of the prostate that are confined to the organ, and in particular incidental pros tatic carcinomas (stage Tla-b), are different.…”

“…One hundredpercent (17) of these were detected by transrectal ultrasound, while 41 percent (seven of 17) were detected by digital rectal examina tion. Eighty-two percent (14) were sur gically staged, and all 14 had negative lymphadenectomies. Of thesixwho elected radical prostatectomy, only one (17 per cent) was upstaged.…”
