Transseptal course of anomalous left main coronary artery originating from single right coronary orifice presenting as unstable angina

Abstract: Transseptal course of coronary artery has often been described as a benign entity; however, this report and literature analysis provides growing evidence of high risk of serious cardiovascular events in this anomaly. We present a case of unstable angina in a patient with anomalous common origin of left and right coronary arteries from a single coronary ostium at the right sinus of Valsalva, with subsequent transseptal course of the left main artery, review of relevant literature, and discussion of possible man… Show more

“…The two patients with unstable angina were a 51-year-old male 9 and a 62-year-old female. 10 Both of these patients had angiographic evidence of clean coronary arteries. The patient who developed recurrent episodes of ventricular tachycardia was 42-years old, otherwise had normal coronary arteries, and had complete resolution of the tachycardia following coronary bypass grafting.…”

“…There are at least three case reports in older patients with an anomalous LMCA originating from a single coronary ostium who presented with either unstable angina or ventricular tachycardia. The two patients with unstable angina were a 51‐year‐old male and a 62‐year‐old female . Both of these patients had angiographic evidence of clean coronary arteries.…”

“…This technique can show the most lifethreatening features of the anomaly and has a high spatial resolution. Including this case, there are 6 cases of the right single coronary artery with a subsequent trans-septal course of the left main coronary artery reported in the literature [6].…”
