Transverse Fracture of the Stapes Anterior Crus Caused by the Blast Pressure from a Land Mine Explosion

Abstract: Stapes fractures without other ossicle problems are rare and ossicle problems due to explosion pressure are also rare. We describe a very rare case of stapes anterior crural fracture resulting from a land mine explosion. As this case suggests, a close examination of the ossicles is necessary during an exploration tympanotomy.

“…Although this kind of injury will seldom be a radiologic diagnosis, tympanic membrane rupture should be kept in mind in regard to patients who are exposed to explosive blasts (Fig 4). The initial blast wave can also cause ossicular fractures and dislocations (20,21). Although less important in the acute phase of injury triage, these injuries can be diagnosed at dedicated thin-section CT of the temporal bone (22).…”

“…In the present case, a distortion force may have induced fracture of the posterior crus of the stapes, as also reported for stapes dislocations following head injury [5]. In patients with persistent conductive hearing loss and imaging considered to be normal, some authors recommend surgical exploration to demonstrate and simultaneously treat the ossicular lesion [6]. We emphasize the importance of early tympanometry, which may suggest dislocation of the ossicular chain in the presence of increased compliance.…”

“…To our knowledge, no case of isolated fracture of the posterior crus of the stapes has been previously reported. The low incidence of this site can be explained by the anatomy of the stapes, as the posterior crus is protected by the annulus and supported by the stapedius tendon [6]. In the present case, a distortion force may have induced fracture of the posterior crus of the stapes, as also reported for stapes dislocations following head injury [5].…”
