Transverse minilaparotomy for open abdominal aortic aneurysm repair

Abstract: Left upper quadrant minilaparotomy is a feasible minimally invasive approach to open AAA repair. This technique is associated with low morbidity and mortality and short hospital stay, particularly in patients identified through AAA screening.

“…6,25 Furthermore, low incisional hernia rates have also been observed after AAA repair via transverse incisions specifically. 26,27 Future studies could investigate how prophylactic mesh reinforcement via midline incision compares to surgery via other incision directions. With regard to the retroperitoneal approach, only 30 day results were available from a single RCT, which showed no reduction in incisional hernias after this approach.…”

“…Transverse incisions have shown fewer hernias and may have the same exposure of the aorta (Fassiadis et al, 2005). A recent report from the UK showed the feasibility of a minilaparotomy for open aortic aneurysm repair (Hafez et al, 2011). They performed a 10 cm transverse supraumbilical laparotomy in 83 non-obese patients with AAA and reported a low mortality rate of 2.4% and only 2 incisional hernias.…”
