OBJECTIVETo review the literature to determine whether compression therapies and nutrition status influence venous ulcer healing.DATA SOURCEA systematized bibliographic review was carried out by searching PubMed, Scopus, and Cochrane databases for studies published between 2015 and 2020, using descriptors in Spanish and English.STUDY SELECTIONAfter establishing the research question and applying the filters based on inclusion and exclusion criteria, 114 articles were found. After screening, 11 articles were selected for the review: 8 were identified in the literature search, and 3 were added from the bibliographic references of other studies.DATA EXTRACTIONThe authors developed a data extraction sheet that recorded the following variables: author, country, year of publication, level of evidence, research design, therapy type, initial ulcer size, active ulcer history, pain, healed wounds, wound reduction, therapy tolerance, nutrition assessment method, body mass index, and nutrition disorders.DATA SYNTHESISFour studies analyzed the relationship between nutrition status and venous ulcer healing, finding that these patients tend to have a high body mass index; a deficit of nutrients such as vitamin A, D, or zinc; and an excess of lipids and carbohydrates. Seven studies compared different types of compression to determine which provided the best results, with two- and four-layer therapy being the most commonly used. Variables such as small wound size and recent onset were associated with better prognosis.CONCLUSIONSIn terms of nutrition, the parameters analyzed are very limited. Although several studies show that two-layer therapy produces the greatest ulcer healing, there is not enough information to recommend one type of therapy over the other. Therefore, more clinical trials are needed to study broader nutrition parameters and compare the types of therapy under matched conditions to determine their influence on ulcer healing.

Predictive factors for worsening chronic wounds

Objective: to analyze the predictive factors for the aggravation of chronic wounds. Methods: quantitative, cross-sectional study, whose collection was performed using the Bates-Jensen Wound Assessment Tool. For data analysis, the independent t test, analysis of variance and multiple linear regressions were applied. The level of significance adopted was 0.05. Results: in the bivariate analysis, it was observed that the variables making use of tobacco (p=0.005), presenting food restriction (p=0.001), signs of infection (p=0.005), odor characteristics (p<0.001) and evaluation pain (p=0.012) contributed to the worsening of chronic wounds. Conclusion: it was found as predictors for the worsening of wounds: using tobacco, having food restriction of the hypo sodium or hypoglycemic type, in addition to presenting edema in the surrounding area, as a sign of infection.

Las úlceras por presión (UPP) son lesiones en la piel y el tejido inferior que resultan de una presión prolongada sobre la piel que recubre las partes óseas del cuerpo, como talones, tobillos, caderas y coxis. El riesgo es mayor si está recluido en una cama, utiliza una silla de ruedas o no puede cambiar de posición. Las úlceras por presión prolongan la estancia en el hospital o en el centro sanitario y pueden ser mortales si no se tratan. Con el fin de proporcionar una información completa acerca de las Ulceras por presión, resulta de interés hacer un sondeo acerca de las publicaciones actualizadas acerca de esta afección a través de la revisión bibliográfica de publicaciones científicas y académicas disponibles en repositorios, libros, revistas científicas y páginas web especializadas en el ámbito médico. La bibliografía consultada mantiene la idea de que la clave para abordar las UPP permanece en la valoración de los factores de riesgo de ulceración. Las úlceras por presión no cicatrizan a menos que las causas de fondo sean tratadas de forma eficaz. Una valoración general debe incluir la identificación y el tratamiento efectivo de la enfermedad, los problemas de salud, el estado nutricional, el grado de dolor y los aspectos psicosociales que puedan haber situado a la persona en riesgo de desarrollar UPP. La escala de Braden y la escala de Norton son las más comunes a utilizar porque su especificidad supera el 60%. Las pruebas imprescindibles de laboratorio incluyen: hemograma, coagulación, VSG y bioquímica completa En general, puede plantearse que las úlceras grados I y II serán de tratamiento clínico, mientras que las de grados III y IV serán de tratamiento quirúrgico. El tratamiento nutricional en la cicatrización de úlceras crónicas es importante. Entre las técnicas y tratamientos utilizados se encuentra la técnica de cura húmeda el estimulador eléctrico Stimul W® utilizado con el objetivo de prevenir y acelerar la cicatrización de úlceras dérmicas de diversa etiología patentado y registrado en Cuba y en otros países. Y el uso de sevoflurano con comunicaciones de experiencias clínicas favorables a su empleo tópico en heridas complejas.
