Traumatic acute subdural hematoma and coma: retrospective cohort of surgically treated patients

Abstract: Background: A subdural hematoma is defined as clot formation in the subdural space after vessel rupture or brain parenchyma damage. Several demographic and tomographic factors were associated to poor prognosis, although some debate according to their specific roles still remains. Methods: Retrospective cohort study of comatose patients admitted to a single-institution, tertiary hospital center, between the years 2013 and 2019 with traumatic acute subdural hematoma requiring surgical evacuation were studied… Show more

“…The small number of females might be misrepresentative of this gender; however, this limitation has been observed in the majority of prior studies. The outcome after TBI might be influenced by patient's comorbidity such as diabetes mellitus, hypertension, dyslipidemia, smoking, chronic obstructive pulmonary disease, and heart disease; however, our data could be unique in terms of patient age in comparison to prior studies from other countries [ 14 18 ] as the mean age in our cohort is 34 ± 14 which precludes the presence of high proportion of comorbidities. However, diabetes mellitus and hypertension were found in 11.8%, and multivariate analysis showed that they did not have a significant predictor role for mortality.…”

“…This higher rate of acute traumatic SDH among males than females was also reported in many previous studies from different countries. For instance, the proportions of affected males were 52% (Portugal), 53% (Italy and Spain), 64% (Japan), 65% (the USA), and 74% (Brazil) [ 14 18 ]. However, there is no reported explanation in the literature for such gender discrepancy.…”

“…In general, ultra-early surgery was reserved to patients with worse clinical and radiological findings at arrival, which is known to strongly influence the outcome [ 17 , 28 , 29 , 34 ]. Moreover, the clinical and radiological scenarios of the ASDH patients are dynamic and heterogeneous, with a number of patients who will maintain stable conditions and others who will suffer a worsening, which will eventually influence the timing of the surgery and post-operative outcome [ 35 ].…”

“…Bocca et al in 2021 published a retrospective study regarding patients with acute subdural hematoma who were managed surgically [ 15 ]. Their results were quite alarming as more than half of the patients died and patients who were more than 75 years old had 100% mortality even after surgical management.…”
