Traumatic neurological injuries of the upper limb are common conditions in traumatology and neurosurgery. The disabling nature of these injuries requires urgent diagnosis, medical-surgical therapy and adequate functional rehabilitation techniques, in order to ensure better recovery of function in the medium and long term. The aim our study was to determine the epidemiological and clinical aspects of traumatic neurological injuries of the upper limb and to describe their functional evolutions in rural areas. We conducted a descriptive cross-sectional study (over a period of 5 years and 8 months, from 1st January 2015 to 10th August 2020) at the Saint Jean of Malte Hospital of Njombe (SJMH). This preliminary study identified clinical data before and after treatment, from the medical records of patients and with the help of the Chanson and Michon classification modified by Amend we evaluated their evolution. Out of 468 upper limb traumas the prevalence of traumatic neurological injuries was 10.47%. The median age was 19 years with interquartile range of 38 years, the male sex was predominant with 39 males and 10 females. The ratio M/F was 3.9. The main etiological circumstances were; childbirth (36.74%) and road accidents (20.41%). Fractures were the most common associated injury with 51.02%. Lesions of the brachial plexus were the most common with 42.86%, followed by digital collateral nerves damage with 22.45%. Axonal injuries combining neurotmesis (34.69%) and axonotmesis (30.62%) were the most common type of traumatic neurological injury. The post-traumatic functional recovery reassessment time ranged from 2 weeks to 5 years. The clinical evolution of the sensitive and motor functions was good despite a therapy essentially based on rehabilitation techniques. In this context of limited technical facilities, prevention methods remain the best means of reducing the incidence of these neurological lesions.

Objective This study aimed to estimate the prevalence of brachial plexus injuries and to characterize clinically and epidemiologically patients with brachial plexus injury. Materials and Methods In this cross-sectional descriptive study, 2,923 medical records of patients aged 1 to 64 years who presented at outpatient peripheral nerve unit of the Orthopedic Surgery Department of Hospital Roosevelt, Guatemala, from January 2017 to December 2017, were prospectively analyzed to identify the prevalence and factors associated with brachial plexus injuries. Results The prevalence rate of brachial plexus injuries in patients was 5.74%. This injury is more common in men (90.5%) aged 24 to 64 years. Brachial plexus injuries occurred secondary to motorcycle accident in 72% of the cases, with the majority affecting the dominant upper extremity. In addition, 64.28% of the patients took 1 to 6 months to seek consultation, whereas only 16.07% requested medical assistance <1 month from the onset of symptoms, and this result was associated with early diagnosis and adequate recovery during follow-up. Furthermore, 66.67% presented upper brachial plexus injury with no associated fractures or vascular injury, manifesting distress while performing daily activities that required hand, arm, and elbow movements. Conclusion The risk of suffering BPIs in Guatemala increases in economically active male patients that use motorcycles as main mode of transportation. Patients should consult immediately after injury onset to optimize management results. For this reason, hospitals must develop specialized clinical guidelines to speed up the identification and treatment of BPI injuries.

Peripheral nerve regeneration following injury is often slow and impaired, which results in weakened and denervated muscle with subsequent atrophy. Human Wharton’s jelly mesenchymal stem cells (hWJ-MSC) have potential regenerative properties which, however, remain unknown in mouse nerve recovery. This study investigated the effect of the topical application of hWJ-MSC onto repairing transected sciatic nerves in a mouse model. Human adipocyte-derived stem cells (hADSC) were used as a positive control. The sciatic nerve of BALB/c mice was transected at a fixed point and repaired under the microscope using 10-0 sutures. hWJ-MSC and hADSC were applied to the site of repair and mice were followed up for 1 year. The hWJ-MSC group had significantly better functional recovery of five-toe spread and gait angles compared with the negative control and hADSC groups. hWJ-MSC improved sciatic nerve regeneration in a dose-dependent fashion. The hWJ-MSC group had a better quality of regenerated nerve with an increased number of myelinated axons throughout. hWJ-MSC appear to be safe in mice after 1 year of follow-up. hWJ-MSC also expressed higher levels of neurotrophic factor-3, brain-derived neurotrophic factor, and glial-derived neurotrophic factor than hADSC. hWJ-MSC may promote better nerve recovery than hADSC because of this upregulation of neurotrophic factors.
