Traumatic Retropharyngeal Hematoma in a Patient Taking Clopidogrel

Abstract: The development of a retropharyngeal hematoma may lead to acute airway compromise requiring emergent airway stabilization. We describe the development of a retropharyngeal hematoma in an elderly woman who sustained a fall from standing approximately 10 hours prior to symptom onset who was taking the antiplatelet agents clopidogrel and aspirin. This delayed onset of rapid airway compromise secondary to a retropharyngeal hematoma following a fall in a patient taking clopidogrel is an unusual and potentially life… Show more

“…Bleeding in this space is rare; however, it may cause airway obstruction and sometimes requires emergency intubation. Most reported cases have been related to head and neck trauma, use of anticoagulants, or underlying bleeding diathesis [1][2][3]. In our case, two mechanisms of injury were suspected.…”

“…Retropharyngeal hematoma is a potentially life-threatening condition due to the risk of airway obstruction. Previous reports have suggested causative factors, such as head and neck trauma, the use of anticoagulants, or an underlying bleeding diathesis [1][2][3]. Herein, we report a case of retropharyngeal hematoma in a patient with chronic alcoholism for whom we could not confirm any predisposing factors at the time of examination.…”

“…9 The literature describes retropharyngeal haematomas associated with warfarin, clopidogrel and aspirin, especially in the context of trauma. 5,10 However, to the best of our knowledge, this is the first reported case of an atraumatic retropharyngeal haematoma associated with indomethacin.…”

“…There are two main reasons of RH: trauma and bleeding diathesis. [4] While a history of trauma is the most frequent reason, spontaneous RHs may also occur, typically in those with bleeding diathesis due to genetic disorders or oral anticoagulants. [5] Less than seventy cases have been reported in the literature on airway obstruction due to RH with isolated trauma and no spine injury.…”
