Traumatic rupture of the ileum

Abstract: Three cases of patients who sustained rupture of the ileum following blunt trauma to the abdomen are reported. T h e onset of symptoms was delayed for a period which varied between 5 and 12 hours after the injury.As so many of these patients are either asymptomatic or have minimal symptoms for the first 6 hours after the injury it might be advisable to admit all cases of abdominal injury to hospital for observation.Laboratory investigations of the haemoglobin, white-blood count, and serum amylase contributed n… Show more

“…In the duodenal loops included in their series the fluidfilled closed-loop specimen sustained a blow-out perforation whilst the empty open-ended specimen had only a sero-muscular injury. These closed-loop blow-out lesions are commoner in the proximal jejunum or distal ileum (Evans, 1973) but it would seem possible that this mechanism of injury could produce a duodenal perforation when the fluid-filled closed-loop situation was present. It has also been postulated that a hydraulic blow-out may occur when gastric contents are propelled under pressure through the open pylorus and strike the duodenal wall with enough force to rupture it.…”
