Traumatic tibialis anterior tendon rupture: treatment with a two-stage silicone tube and an interposition hamstring tendons graft protocol

Abstract: A novel technique for managing ruptured tibialis anterior tendon complicated by infection and tendon substance loss in a young adult is described. A two-stage reconstruction technique with a silicon tube and tendon autograft was performed. At first, after local control of the infection, scar excision and placement of a silicone tube was performed. Ten weeks later, ipsilateral hamstrings tendons were harvested and bridged the 7 cm tendon gap. Eighteen months later, the patient has excellent clinical and functio… Show more

“… 3 However, defects >4 cm have been addressed with tendon grafting, whereas defects >10 cm have been successfully treated with gracilis transfer. 1 , 4 , 5 Although more commonly used for upper extremity tendon reconstruction, free functional muscle transfer could be considered. However, this method relies on adequate soft-tissue coverage and a local environment that allows for smooth tendon gliding.…”

“…Tendon autograft is a classical option for tendon reconstruction but usually associated with postoperative adhesions . Two‐stage reconstruction using silicone tube leads to better outcomes . The main disadvantages of this technique are the potential infectious risk and the two‐stage procedure.…”

“…This optimal timing of nerve regeneration is synchronous with the proper time needed for maturation of the pseudosheath. In clinical practice, the minimum time interval between two stages of tendon graft is 2–3 months to permit structural maturation of the pseudosheath ( 35 , 36 ). However, electron microscopy in an experimental study on hen shows that after 4 weeks, a pseudosheath comparable to normal parietal sheath is formed around the silicone tube ( 37 ).…”
