Treatment and toxicokinetics of acute pediatric arsenic ingestion: Danger of arsenic insecticides in children

Abstract: Recovery from arsenic poisoning was attributable to the restoration and maintenance of adequate cardiac output and renal perfusion in early shock, which allowed depot intramuscular British anti-Lewisite to circulate and eliminate the poison. Although an intravenous antiarsenical chelating agent would be advantageous in treating shock from arsenic poisoning, none is currently available. We urge the immediate use of British anti-Lewisite therapy on patient presentation with suspected toxic arsenic ingestion.

“…On one hand, exposure to inorganic arsenite can induce adverse health effects like hyperkeratosis, hyper-pigmentation, cardiovascular disease, reproductive defects, and cancers of several tissues [4648]. On the other hand, arsenic trioxide is considered to be valuable because of its induction of apoptosis in leukemia and pancreatic cancer cells [49, 50].…”

“…A 22-month-old boy ingested approximately 50% of the gel bar from a discarded opened ant bait trap of Grant's Kills Ants ® (0.46% arsenic trioxide) (1). He had few symptoms and an initial total serum arsenic concentration of 1,700 mcg/L and spot urine arsenic concentration of 2,085 mcg/L.…”

“…In 1988 the U.S. Environmental Protection Agency (EPA) restricted the use of pesticides containing inorganic arsenic by banning arsenate (As 5+ ) from use in pesticides (1). However, trivalent arsenite (As 3+ ), as arsenic trioxide (As 2 O 3 ), is still available for use in ant baits.…”

On the beneficial side, arsenic trioxide has been valued because of its induction of apoptosis in pancreatic cancer cells ( via activating the expression of GADD45 proteins [217]).…”
