Treatment efficiency and stability of skeletal Class III malocclusion with a surgery‐first approach

Abstract: Using SFA for skeletal Class III malocclusions saves approximately 6 months of treatment time over 3-step treatment; the stability of the segmentation group was comparable to that of the non-segmentation group, a result that is possibly associated with the fixation of 2 miniplates.

“…Among the 5 identified outcome domains, 2 are over-represented and 3 are under-represented [ Figure 2]. [47] Retrospective n, 37, females 24.0±4.9 years [51] Retrospective study 5 patients (19)(20)(21)(22)(23)(24)(25)(26) Furthermore, this scoping review did not identify any RCT within the SFOA research, indicating that more pertinent research encompassing RCTs are required to arrive at formulating COS. However, the fact that many prospective studies are being carried out is itself promising in nature, and in the future, this will allow researchers and readers to make best use of the available reported trails to formulate the research question.…”

“…Various problems have been reported for this approach. The long duration of orthodontic treatment especially in the presurgical phase may discourage the patients and negatively affect their compliance [18]. Dowling et al reported that the length of conventional surgical-orthodontic treatment is about 22 months and includes 16 months of presurgical-orthodontic treatment and six months of postsurgical-orthodontic treatment [19].…”

“…However, these time periods may vary depending on the operator's skills [20]. Some other disadvantages have also been reported for this approach including the gingival recession, root resorption, and worsened facial esthetics, which may occur during the presurgical-orthodontic phase [18,21]. To prevent them, researchers have suggested a two-step orthognathic surgical approach that does not require preoperative orthodontics [18,[22][23][24].…”

“…Without presurgical orthodontics, the patients may have likely to develop unstable occlusion after surgery leading to relapse. However, some of the comparative studies between conventional and surgery first approach showed no statistical differences in relapse and almost equal for those achieved using the more traditional orthodontics-first approach [17,19,51,53,[59][60][61][62][63]. Advancement of fixation system enabled more stabilized results due to more stable fixation of bony segments.…”
