Treatment of Cerebral Aspergillosis with Itraconazole: Do High Doses Improve the Prognosis?

Abstract: We describe a 72-year-old woman with chronic asthma who presented with cerebral abscesses due to Aspergillus fumigatus after she received treatment with corticosteroids. Therapy with high-dose itraconazole (800 mg/d for 5 months, followed by 400 mg/d for an additional 4.5 months) resulted in complete resolution of all lesions. Serum concentrations of the drug ranged from 2 micrograms/mL to 30 micrograms/mL. Review of 20 cases of cerebral aspergillosis that were treated with itraconazole revealed that three of … Show more

“…In a review by Sánchez et al (1995), three of four patients with cerebral asper- gillosis that were treated with a high dose of ITCZ (800 mg/day or 16 mg/kg/day) survived, while seven of eight patients treated with 400 mg/day ITCZ died. Consistent with those results, the conventional dose of ITCZ failed to control intracranial aspergilloma in our two cases, while the high dose of ITCZ resulted in improvement.…”

“…Recent reports indicate that an extremely high blood concentration of ITCZ that is achieved with a large oral dose may improve the prognosis (Kloss et al 1991;Sánchez et al 1995;Imai et al 1999).…”

“…In general, however, higher concentrations appear to be desirable. Recent reports have recommendedthe use of high dose ITZ for highrisk patients with cerebral aspergillosis for whomconventional therapy has failed (3,17). On the basis of our experience, we strongly recommend high doses of ITZ combined (if necessary) with radical surgery for the treatment of advanced cerebral aspergillosis.…”

“…This form of cerebral aspergillus infection has a very poor prognosis in spite of intracranial surgery and conventional antifungal therapy. Recent reviews suggest that high-dose itraconazole (ITZ) is a suitable treatment for cerebral aspergillosis (3). Wereport a case of a skull base aspergillus infection which originated from the sphenoid sinus.…”
