Treatment of early-onset schizophrenia

Abstract: Although psychosocial interventions are an important adjunctive treatment, antipsychotic medications continue to be the mainstay of treatment. Careful monitoring of metabolic side effects and age-appropriate intervention is particularly important, as children and adolescents appear to be more likely to develop metabolic abnormalities such as pronounced weight gain, which may significantly impact adherence as well as lead to other health issues.

“…Notably, these patients were adolescents with early-onset schizophrenia. Most were diagnosed by age ≤13 years, constituting a sample with early age of onset even in comparison to other adolescent schizophrenia studies (Mattai et al, 2010). This analysis demonstrated a significant, albeit modest, correlation of paternal age with treatment response in adolescents with schizophrenia, but not with the age of onset or initial symptom severity in the offspring.…”

“…During the DB acute treatment period, patients were randomized (1:1 ratio) to receive once-daily paliperidone ER orally ( [5][6][7], flexibly dosed 5, 10, or 15 mg per day from day 8 to EOS). The randomization was based on a computer-generated randomization schedule balanced by using permuted blocks of treatments and stratified by center.…”

“…4 An earlier age of onset for schizophrenia, particularly prepubertal, is associated with a poorer prognosis and greater impact of the disease on personality and relationship development, cognitive functioning, educational and work attainment, and social functioning. 1,5 Moreover, adolescent-onset schizophrenia is a lifelong illness.…”

“…When the disorder begins in early adolescence (under 18 years old) it is termed early onset schizophrenia (EOS), with some cases beginning even earlier (under 14 years old) and called child onset schizophrenia (COS) or very early onset schizophrenia (VEOS) [1][2][3] . EOS is a rare condition and it is estimated to account for less than 4% of the cases of schizophrenia 4 .…”
