Treatment of femoral fractures after hip prosthesis

Abstract

Zusammenfassung: Im Zeitraum von 1995 bis 1997 wurden in unserer Klinik 24 htiftprothetische Operationen durchgeftihrt mit Implantation eines Prothesenspezialschaftes aufgrund erheblichen Knochenverlustes sowie aufgetretener Frakturen des proximalen Femurs. Bei sechs Patienten erfolgte ein prim~irer Prothesenwechsel, sechsreal lag bereits pr~ioperativ eine Femurfraktur bzw. eine Fraktur der bisherigen femoralen Prothesenkomponente vor, bei ftinf Patienten war es im Rahmen der Wechseloperation zu Frakturen gekommen. Pathotogische Frakturen lagen ftinfmal vor, einmal erfolgte beim Prothesenwechsel eine femorale Korrekturosteotomie, einmal wurde eine Pseudarthrose bei implantierter Prothese behandelt. Defekte des Acetabulums lagen nicht vor, eine Revision der Hiiftpfanne war in keinem Fall erforderlich. Die Prothesenwechsel erfolgten mit einer speziellen Schaftkomponente (ESKA-Implants, Ltibeck) mit elastischer Stielverliingerung und distaler Verriegelungsm6glichkeit. Postoperativ wurde nach frtihzeitiger Mobilisation und vortibergehender Teilbelastung des operierten Beines naeh etwa drei Monaten distal entriegelt und die Vollbelastung aufgebaut. Die Nachuntersuchung der bisher nach diesem Verfahren durchgefiihrten ProthesenwechseI ergab bei allen Patienten einen guten Prothesensitz nach distaler Entriegelung. Die elastisch stielverlfingerte Femurschaftkomponente soll als biomechanisch gfinstiges Implantat zur h(iftprothetischen Versorgung, vor allem bei periprothetischen bzw. w~ihrend geplanter Wechseloperationen aufgetretener Femurfrakturen und h6hergradigen Femurdefekten, vorgestellt werden.Abstract: Between 1995 and 1997 we operated 24 patients with implanted hip prosthesis and periprosthetic fractures of the proximal femur using a revision-prosthesis with elastic stem. In 6 patients we performed a primary revision due to aseptic loosening and massive bone loss. In 5 cases fractures occurred intraoperatively during prosthesis revisions, in 6 cases fractures of the prosthesis and/or femur existed preoperatively and in 5 patients we saw pathologic femur fractures. In one patient we performed an osteotomy and correction of the femur axis during prosthesis revision and in another patient we removed a broken stem caused by femur pseudarthrosis. No acetabular revision had to be performed. The used prosthesis stem (ESKA-lmplants, Ltibeck) was made of a proximal massive and a distal elastic part with the possible interlocking screw placement. During postoperative mobilisation partial weight bearing was allowed for 3 months, then full weight bearing was aimed after removal of the interlocking screws. In all treated patients we saw a good prosthesis ingrowth. We present the elastic stem as good implant, especially in case of femur fractures or bone stock loss.
