“…Since then, several techniques and fixation methods have been reported related to metatarsal osteotomy in Freiberg disease. 9,12,13,20 Known complications in patients with osteotomy include fixation failure and metatarsalgia. 1,23,28 In a study by Ikoma et al, 13 13 patients with Freiberg disease had extraarticular osteotomy, and after a follow-up period of a mean 17 months, dorsiflexion was seen to have increased from a mean of 37.2° preoperatively to 73.6°.…”

“…Helix-Giordanino and colleagues performed the first osteotomy described for Freiberg’s disease, Gauthier Osteotomy, in 30 consecutive cases and reported good patient satisfaction and AOFAS scores postoperatively. 11 Lee and coworkers reported significant improvements in postoperative VAS and AOFAS scores among patients undergoing Weil and Dorsal closing wedge osteotomy for Freiberg’s disease of the second metatarsal head. 15 In the later stages of this disease process (stage IV and V), it is difficult to restore joint congruity, and therefore joint arthroplasty has been recommended.…”

“…9,27 Even in these early stages, most authors have focused on osteotomy surgery, rotating the metatarsal head dorsally in order to take the dorsal diseased head out of the central arc of motion and instead bringing the plantar cartilage, which is generally intact, into this articulation. 11,15 This is a challenging procedure and risks alteration of the normal loading biomechanics of the forefoot. This can be desirable when shortening of the metatarsal is feasible.…”

“…While most articles researched showed the mean ages during adolescence and early adulthood, interestingly, a study by Helix-Giordanino, et al had a mean age of thirty patients with Freiberg's infraction of 61 years. That article does not focus on the advanced age of the group and its impact, rather it concludes that a dorsal closing wedge osteotomy is the best way to surgically treat it [2]. Freiberg's infraction (Figure 2 and Figure 3).…”
