Treatment of Hypochondriasis in Two Schizophrenia Patients Using Clozapine

Abstract: Hypochondriasis (HYPO), an obsessive-compulsive spectrum disorder, is frequent in patients with schizophrenia (SCH) (20%), especially among those treated with clozapine (36.7%). Treatment options for OCS/OCD in patients under clozapine (CLZ) include combining clozapine with amisulpride/aripiprazole or a mood stabilizer, augmenting clozapine with a serotoninergic reuptake inhibitor, adding cognitive behavioural therapy, and gradually reducing dosage. No treatments have been proposed for HYPO in patients using c… Show more

“…Of the 59 cases where antidepressants were added without simultaneously changing the dose of clozapine or adding other medications to treat OCS, 29 patients (49.2%) improved [17, 19-21, 27, 29, 30, 32-34, 36, 39, 48, 53, 60, 64, 65, 70, 72], whereas 30 patients (50.8%) demonstrated inadequate response to an initial trial of SSRI/ clomipramine [20,22,25,35,37,42,43,45,46,49,50,54,55,59,60,63,[66][67][68][69]71].…”

“…Twenty-five non-responders underwent additional trials to treat their OCS. Eleven non-responders underwent trials with different antidepressants [22,35,37,42,55,59,60,[67][68][69]71], of whom only 2 patients responded (18.2%) [37,67]. Sixteen (64.0%) non-responders showed improvement without psychotic exacerbation following a decrease in the dose of clozapine [42,45,46,49,55,59,60,66,68], of whom 8 (50.0%) were concomitantly treated with aripiprazole [49,55], 2 with antidepressants [59,66], 2 with both aripiprazole and antidepressant [42,45], and 1 with valproic acid [46].…”
