Treatment of Infected Total Hip Arthroplasty

Abstract

인공 고관절 주위 감염은 환자를 황폐화 시킬 수 있는 심각한 합병증이다. 이 합병증은 고관절 기능의 현저한 감 소는 물론, 진단의 불확실성, 항생제의 장기간 투여, 막대 한 치료비용, 재수술의 위험성 등으로 발생해서는 안 되는 것이기에 모든 병원이 인공관절 치환술 후에 합병증을 줄 이기 위해 노력하고 있다. Charnley와 Eftekhar 1) 는 초기 인공관절 술 후 감염률이 8.9%이었으나, 수술실 및 수술 팀의 철저한 소독, Laminar air flow system, 방수성 소독 포 등의 사용으로 1%까지 그 발생률을 낮추었다고 발표 하였다. 지금까지 가장 낮은 감염률을 보고한 것은 British Medical Research Trial의 0.3%이다 2) . 감염의 원 인에 대한 광범위한 지식과 예방법, 기술적 진보, 수술 수 기의 개선 등에도 불구하고 이보다 낮은 감염률을 이루어 내지 못하고 있다. 감염균이 인공관절 주위 공간에 도달하는 경로는 4개가 있다. 첫 번째는 수술도중 감염으로 잘 알려진 인자이다. Lidwell 등 3) 은 공기 오염의 감소로 인공관절 감염 발생률 이 점진적으로 감소 추세를 보이고 있다고 하였다. Salvati 등 4) 도 horizontal ultraclean air laminar flow를 이용하여 감염률을 1.4%에서 0.9%로 감소시켰다고 보고하였다. 두 번째 감염 경로인 직접적 혹은 인접한 곳에서의 확산은 표 재 감염이 심부로 진행하거나 또는 요도 협착에 의한 회음 부 농양에 의한 고관절 감염이다 5) . 세 번째 감염 경로는 혈 류를 통해 피부 감염, 치과적 감염, 요로 감염 등을 통해 균이 전파되는 것이다 6) . 네 번째 감염 경로는 이전에 감염 된 적이 있던 경우 균이 재활성화 되는 것이다 7) . 인공관절 감염의 분류 Tsukayama 등 8) 은 인공관절 감염을 4단계로 분류하였 다. 1단계는 술 중 여러 군데에서 시행한 균동정(culure) 결과상 최소 두군데 이상에서 양성이 나온 경우를 말한다. 2단계는 수술 후 조기 감염(early postoperative infection) 으로 수술 후 한달 이내에 일어나는 것을 말한다. 고열과 오한, 패혈증, 지속적인 통증 등 전신 증상과 함께 누공, 발적, 종창, 농양 등의 국소적 증상이 동반된다. 3단계는 만성 지연 감염(late chronic infection)으로 수술 후 한달 이후에 증상이 발현하는 것을 말하며 증상이 서서히 진행 되고 그 정도로 심하지 않다. 4단계는 급성 혈행성 감염 (acute hematogenous infection)으로 다른 부위의 감염 이 혈행을 통해 전파되거나 증상의 발현이 갑자기 일어나 는 경우가 많다.Deep infection after total hip arthroplasty is one of the most serious post operative complications. Treatment of infection of the hip joint can be made very difficult by poor response to antibiotic therapy. Infection rates of total hip arthroplasty have decreased due to prophylactic antibiotics, sterilization of surgical instruments, cleaner operation environments, Improvement of surgical methods and proper patient selection. Recently, post operative infection rates of primary or revision arthroplasties were reported to still be as high as 1% to 2%. Therefore, the aim of this article is to review the recent literature and to evaluate the cause, environment diagnosis and treatment of infection after total hip arthroplasty.

When patients develop fever three days following surgical treatment of the infection, orthopedic surgeons may first consider an inadequate control of the initial infection or an infection relapse and perform variable tests to determine other causes of fever5).…”
