Treatment of poikiloderma of Civatte with the pulsed dye laser: a series of patients with severe depigmentation

Abstract: Great care is needed when PDL treatment is used for poikiloderma of Civatte. Pigment changes have been incidentally mentioned as late complications but have not been well documented as the late depigmentation has been in this series. It is advisable to use fluences as low as possible, and not exceeding an upper limit of 5 J/cm2, on a 10-mm spot size. More research is needed to define an optimal pulse duration.

“…Using high fluence on the PDL can result in late depigmentation which may be as distressing as the original condition [ 20 ]. Some authors have reported very high efficacy and low incidence of side effects after treating poikiloderma of Civatte with IPL [ 21 ].…”

“…Results in clinical trials using the vascular pulsed dye laser for the treatment of erythema in rosacea, blood vessel dilation/growth in hemangiomas and telangiectasia, as well as hemosiderin deposition from capillaritis are excellent; however, purpura, prolonged erythema, late-onset hypopigmented macules, and post-inflammatory hyperpigmentation are complications that require serious consideration prior to therapy, especially in the patient with cosmetic concerns 2731. Although patients are often dissatisfied knowing that there are no “quick fixes” available, it is important that there is a clear discussion regarding the planned therapeutic regimen and the importance of close clinical follow-up, and that meticulous commitment to treatment be emphasized by the practitioner.…”

“…Numerous lasers and light sources including: pulsed‐dye lasers (PDLs), intense pulsed light sources (IPLs), long pulse‐duration neodymium‐doped yttrium‐aluminum garnet (Nd:YAG) lasers incorporating a potassium‐titanyl‐phosphate (KTP) frequency‐doubling crystal, and fractionated non‐ablative and ablative lasers have all been used to remove the extra blood vessels and pigment comprising POC. The intense vascularity of POC makes this an ideal condition to treat with vascular specific lasers such as the PDL and KTP lasers.…”
