Treatment of Refractory Postural Tachycardia Syndrome with Subcutaneous Octreotide Delivered Using an Insulin Pump

Abstract: Postural Tachycardia Syndrome (PoTS) represents a disorder of the autonomic nervous system that results in symptoms of orthostatic intolerance. Despite having a severe impact on the patient's quality of life, the current treatment options for PoTS are based on limited evidence. Subsequently, this results in clinicians having to utilise a variety of treatment regimens in the hope of successfully providing symptomatic relief. However, the options available for PoTS are not without significant side effects that c… Show more

“…In specialized centers, patients with significant postprandial tachycardia are occasionally treated with octreotide, a somatostatin analog, used to reduce splanchnic pooling. 73 , 74 However, the need for subcutaneous or intramuscular administration, as well as the cost and side effect profile of octreotide have limited its use. A small case series on sinus node modification has found no significant benefits.…”

“…The clinical significance of POTS is increasingly appreciated due to the recent reports regarding the comorbidities of POTS, such as depression [1], sleep disorders [6], chronic fatigue syndrome [7], and diminished quality of life (QOL) [6,8]. While the long-term prognosis of POTS has not been investigated well, some patients suffer from refractory POTS [9][10][11].…”

“…Among them, eating smalls meals throughout the day and the extremely empirical use of the somatostatin analogue, octreotide, that suppresses the secretion of all vasodilatory peptides but the risk of inducing malabsorption syndrome. 7,8 These interventions, however, are not supported by any robust scientific evidence. Therefore, understanding the pathophysiology behind the worsening postprandial symptoms in POTS, with the goal of developing targeted therapies, is of the utmost importance.…”
