Treatment of Sexually Transmitted Bacterial Diseases in Pregnant Women

Abstract: Testing for and treating sexually transmitted diseases (STDs) in pregnant women deserves special attention. Treatment possibilities are limited because of potential risks for the developing fetus, and because effects can differ in pregnant compared with non-pregnant women, re-infection may be missed because of the intrinsic delicacy of contact-tracing during pregnancy and because pregnant women are more reluctant to take the prescribed medication in its full dose, if at all. However, the devastating effects of… Show more

“…BVAB have been reported in HIV-positive women with BV [65] and are considered to be acquired from external reservoirs such as the anus [66]. Also associated with BV infection in HIV-positive women are M. hominis and a combination of Bacteroides , Prevotella , Leptotrichia , Atopobium and Gardnerella [49,67,68,69,70,71,72] known to attract CD4 cells to the mucosa, with a concomitant reduction in lactobacilli [67]. Among the Gram-positive anaerobic cocci known to colonise the vagina are Peptococcus , Peptostreptococcus , Peptoniphilus and Anaerococcus .…”

“…Azithromycin has been used extensively to treat sexually-transmitted and other bacterial infections during pregnancy in total treatment doses of up to 1–2 g and is considered a safe alternative for the treatment of chlamydia, mycoplasma and group B streptococci in pregnant women who are allergic to beta-lactam antibiotics [71-73]. …”

“…Sexually-transmitted infections (STIs) are common in women of childbearing age in Africa and may go undiagnosed and untreated during pregnancy[127]. Monthly azithromycin reduces STI incidence significantly in high-risk groups[128] and is safe and effective for treating gonorrhoea as well as chlamydia infection during pregnancy[71], which is associated with increased risk of stillbirth, prematurity and LBW[129]. Mefloquine-azithromycin may, therefore, represent a promising, safe and effective combination for IPTp and should be considered urgently for evaluation in phase III clinical trials.…”

“…Unfortunately, there is a paucity of approaches to the treatment of opioid and cocaine dependence developed to address the unique challenges presented by drug dependent pregnant women 47. A particular concern with substantial significance to the health of both the pregnant mother and the fetus concerns the degree to which pregnant women with drug dependence exhibit sexual or injection risk behavior that may predispose them to contract HIV8 or other infectious diseases including sexually transmitted infections, such as syphilis, gonorrhea, and trichomonas 9, 10…”

“…Thus, extant study findings among drug dependent women in treatment suggests that a co-existing psychiatric symptoms may place women at heightened risk to contract HIV and other infectious diseases by virtue of elevated risk behavior associated with psychiatric disorder status. This relationship between psychiatric disorder status and elevated infectious disease risk is particularly relevant to opioid and cocaine dependent women who are pregnant because both the diseases themselves and the treatments for the diseases may adversely affect the health of both mothers and their fetuses 9, 10, 21, 22…”
