Aim: To evaluate the treatment of endometrial hyperplasia (EH) with different progestins. Methods: Eighty-two women with simple EH without atypia were included. Patients were offered oral progestagens and were randomized to one of three options for 3 months: medroxyprogesterone acetate (MPA, 10 mg/day), lynestrenol (LYN, 15 mg/day) and norethisterone (NET, 15 mg/day) for 10 days per cycle. Patients were reevaluated after treatment. Women diagnosed with proliferative and nonatypical EH at the second curettage were offered the same progestins for another 3 months. The primary outcome of the study was the proportion of women requiring further treatment. Results: Of the 82 women, 46 (56.1%) received MPA (23.2%), LYN (13.4%) and NET (19.5%) therapy for another 3 months at the end of the first 3 months of treatment. The patients receiving MPA showed resolution in 36.7% of the cases versus 37% in the NET group. The highest resolution rate (56%) was observed in the LYN group, although there was no statistically significant difference between progestins regarding the proportion of women requiring further treatment (χ² = 2.608; p = 0.271). Conclusion: It seems that the efficacies of oral progestins are similar at these dosages in simple EH without atypia.

More than 15 case-control studies and at least four large cohort studies demonstrated a decrease in the risk of endometrial cancer of about 50% for ever use of combined oral contraceptives (COCs). In most of these studies, this protective effect persisted for more than 10-15-20 years after cessation of the COC. An increasing protective effect with longer duration of COC use has been found in most studies. The beneficial effect was independent of the composition of COC, i.e. dosage and type of progestogen, combined with ethinyl estradiol 30-50 mg/day. COCs with higher progestogen potency seem to be somewhat more effective. Nonhormonal uterine devices have also been found to be strongly protective; however, data on oral or injectable progestogen-only preparations (POPs) including the levonorgestrel-releasing intrauterine system (LNG-IUS) are still rare, but also suggest similar protective action. COCs, POPs, as well as LNG-IUS can effectively reduce endometrial hyperplasia but should only be used in exceptional cases in patients with or after endometrial cancer. In contrast to nonhormonal IUS, systemic side effects cannot be excluded with LNG-IUS, but they are certainly rare, as the main effect has decreased the endometrial estrogen response because of the high endometrial tissue levels of LNG.

BackgroundAbnormal uterine bleeding may be defined as changes in frequency of menstruation, duration of flow or amount of blood loss. Early accurate diagnosis and proper treatment is essential to prevent progress to endometrial cancer. ObjectiveThis study was done to evaluate the histopathology of endometrium for identifying the endometrial causes of abnormal uterine bleeding. MethodThis was a retrospective study done in department of pathology, Patan Hospital from 1 August 2011-to 31 July 2012; total 413 cases were included in this study. ResultsThe age of the patients ranged from 19 to 70 years. The most common clinical presentation was menorrhagia (47.46%), followed by continuous bleeding (14.04%) and metrorrhagia (13%).The commonest histopathological diagnosis was secretory endometrium 128 cases (30.99%), followed by proliferative endometrium 91 cases (22.03 %). Secretory endometrium was most commonly (52 cases) observed in 41-50 years age group followed by 43 cases in 30-40 years age group. Similarly proliferative endometrium was also most commonly seen in 41-50 age group (44 cases) followed by 31-40 years of age group (32cases). Malignant disease was most commonly seen in 51-60 years age group. ConclusionsAbnormal uterine bleeding is most common in 21-30 years age group.The most common clinical presentation is menorrhagia followed by continuous bleeding. Secretory endometrium is the most common histopathological findings. Malignancy is more common in postmenopausal group.
