BackgroundThe prevalence of pediatric ischemic stroke rose by 35% between 1990 and 2013. Affected patients can experience the gradual onset of cognitive impairment in the form of impaired language, memory, intelligence, attention, and processing speed, which affect 20–50% of these patients. Only few evidence-based treatments are available due to significant heterogeneity in age, pathological characteristics, and the combined epilepsy status of the affected children.MethodsWe searched the literature published by Web of Science, Scopus, and PubMed, which researched non-pharmacological rehabilitation interventions for cognitive impairment following pediatric ischemic stroke. The search period is from the establishment of the database to January 2022.ResultsThe incidence of such impairment is influenced by patient age, pathological characteristics, combined epilepsy status, and environmental factors. Non-pharmacological treatments for cognitive impairment that have been explored to date mainly include exercise training, psychological intervention, neuromodulation strategies, computer-assisted cognitive training, brain-computer interfaces (BCI), virtual reality, music therapy, and acupuncture. In childhood stroke, the only interventions that can be retrieved are psychological intervention and neuromodulation strategies.ConclusionHowever, evidence regarding the efficacy of these interventions is relatively weak. In future studies, the active application of a variety of interventions to improve pediatric cognitive function will be necessary, and neuroimaging and electrophysiological measurement techniques will be of great value in this context. Larger multi-center prospective longitudinal studies are also required to offer more accurate evidence-based guidance for the treatment of patients with pediatric stroke.

Evolutionary psychology has provided a groundbreaking approach to understanding human cognitive architecture during the last few decades, yet this approach has been overlooked when investigating and designing cognitive enhancement therapies. In this article, we posit that understanding why integrated cognitive, emotional, and motor (CEM) systems evolved in the first place can help researchers develop more effective cognitive enhancement strategies. According to an evolutionary perspective, a wide range of CEM systems was designed by natural selection to deal with adaptive problems and maximize reproductive success. However, CEM systems that evolved with a spectrum of problem-solving capabilities may not be as active in modern times due to the environments and challenges in which they evolved no longer being relevant. In exploring this perspective, we propose a theoretical model that explains the evolution of CEM systems and highlights the selection pressures that shaped cognitive-motor mechanisms in ancestral environments. From this viewpoint, and considered in light of current cognitive enhancement strategies and successes from cognitive training, neurostimulation, and physical exercise, we establish a novel framework for evolutionary cognitive enhancement (ECE) therapy. Moreover, toward applying the ECE approach in both clinical and non-clinical contexts, we put forward principles and guidelines for designing ancestral-like environments and suggest methods for recreating adaptive problems in real-world settings.

Objective: To establish reference values for the computerized cognitive test and evaluate cognitive function improvements across different age groups, we introduce the computerized Cognitive Function Test program (eCFT), specifically designed for children. We aimed to establish eCFT reference values and assess cognitive function improvements across different age groups.Methods: We included children aged 3–6 years with confirmed normal cognition based on the Korean Developmental Screening Test for Infants and Children and Kaufman Assessment Battery for Children-II. The eCFT consists of 8 subtests for visual perception, attention, memory, and executive function.Results: A total of 66 participants (36 males and 30 females) with an average age of 4.4 years participated. The age 6 group consistently outperformed both age group 3 and 4 in terms of correct responses. With regard to the completed stage, the “selective auditory stimulus” test findings were 2.0 and 3.9 for the age 3 and age 6 groups, respectively (p<0.05). The “trail-making” test findings were 1.7, 2.1, 2.6, and 2.8, respectively (between ages 3 and 6, p<0.01; between ages 4 and 6, p<0.05); moreover, the age 5 group surpassed the age 3 group (2.6 and 1.7, respectively, p<0.05).Conclusion: The eCFT is an easily accessible tool to evaluate cognitive function in young children. We introduce reference values with a cutoff range for preschool-aged children, enabling early intervention for those with cognitive impairment. Given its accessibility and relatively short evaluation time, the eCFT has potential for clinical use.
