Trend in Instrumental Vaginal Deliveries at the National Referral Hospital in Bhutan: A Review of Hospital Records

Abstract: Instrumental vaginal delivery (IVD) is a core signal function in emergency obstetric and neonatal care. The revival of declining trend in IVDs worldwide against an alarming rise in cesarean section must be viewed in a holistic manner. Every country must devise its own evidence-yet context-based strategies to revive the dying art of this lifesaving obstetric procedure.

“…Further more than nine out of ten patients that had OVD in our cross-sectional study had vacuum (kiwi cup). Our finding is in keeping with other studies in LMICs that showed that birth attendants are likely to choose vacuum because of ease of use, and lack of expertise and supervision to perform forceps delivery [ 3 , 6 , 7 , 12 , 31 , 40 ]. Although Cochrane studies has shown that if forceps is used appropriately, it is likely to be successful.…”

“…The dramatic decline in OVD rates at CHBAH from 2014–2019 may be related to a number of factors. As previously reported, lack of requisite training, skills and supervision to perform OVD, lack of necessary equipment, and fear of litigation are some factors responsible for the declining art of OVD [ 3 , 6 , 7 , 8 , 13 , 30 , 31 ]. In South Africa, the initial guideline for the prevention of Mother to Child transmission of HIV (PMTCT) recommended that OVD, (especially Vacuum delivery) should be avoided among HIV positive women [ 3 ].…”

“…1 Globally, approximately, 10-20% of the women in labor require obstetric interventions or support, with instrumental delivery accounting for 6-12% of these interventions. 2 The World Health Organization (WHO) and United Nations Population Fund (UNFPA) emphasize the importance of assisted vaginal delivery in providing comprehensive emergency obstetric and newborn care. 3 The incidence of OVD varies across countries, with rates is reported as follows: UK: 10-15%; 4 US: 3%; 5 Oman: 3.8%; 6 India: 2.7-7.7%; 7,8 in Singapore: 10%; 9 and in Ethiopia: 10%.…”
