Trends in Statin Use Among US Adults With Chronic Kidney Disease, 1999–2014

Abstract: Background The 2013 American College of Cardiology/American Heart Association cholesterol guidelines recognize cardiovascular disease and diabetes mellitus but not chronic kidney disease ( CKD ) as high‐risk conditions warranting statin therapy. Statin use may be lower for adults with CKD compared with adults with conditions that have guideline indications for statin use. Methods and Results We analyzed data from the… Show more

“…To calculate estimated glomerular filtration rate (eGFR), we adopted the Chronic Kidney Disease Epidemiology Collaboration equation [39]. Briefly, albuminuria was defined as the ratio of urinary albumin to creatinine (ACR) [40]. CKD was defined as ACR ≥30 mg/g or eGFR blow 60 mL/min/1.73 m 2 [41].…”

“…66 Statin medications also remain one of the few proven interventions to prevent incident cardiovascular events in adults with CKD not requiring KRT 67 , yet statin use is especially low (<30%) among adults with CKD in the absence of diabetes or known heart disease. 66,68,69 Studies are needed to increase the utilization of proven therapies, with specific emphasis on disadvantaged populations. Because most kidney disease is never diagnosed until kidneys fail, research is needed to determine efficient methods to identify patients with kidney disease by utilizing electronic health records.…”

“…The American College of Cardiology/American Heart Association (ACC/AHA) guidelines from 2018 define chronic kidney disease as a risk factor for the prevention of atherosclerotic cardiovascular disease and recommend the use of statins in these patients if they are not dialyzed. However, the results of individual studies show a higher incidence of adverse statin effects in reduced renal function, and each patient must be individually approached when considering these START criteria 18,19 . In a significantly smaller number of patients, the absence of angiotensin converting enzyme inhibitors and metformin in therapy was observed.…”
