Trends in the Rates of Peripartum Hysterectomy and Uterine Artery Embolization

Abstract: The objective of this study was to determine the trends in national rates of peripartum hysterectomy (PH) and uterine arterial embolization (UAE) in Korea. We used data collected by the Health Insurance Review & Assessment Service of Korea and analyzed data from patients who gave birth during the period from 2005 to 2008. There were 1785,178 deliveries during the study period, including 2636 cases of PH (1.48 per 1000 deliveries). The PH rate in 2005 was 1.57 per 1000 deliveries and in 2008 it was 1.33 per 100… Show more

“…a The estimated rate of peripartum hysterectomy in the present study was largely consistent with most prior work 6,7,10,31,32 but inconsistent with others. 11,[33][34][35][36] Those studies were singlesite analyses, 11,35,36 included low-risk populations, 34 or were from international cohorts. 33 The finding that rates of peripartum hysterectomy have risen agrees with previous analyses 1,7,37,38 and builds upon this prior work by evaluating recent, race-specific trends in peripartum hysterectomy.…”

“…8,108,[145][146][147] Uterine rupture was the most common indication in lower middle income settings, probably as a result of higher rates of obstructed labor, lack of labor monitoring, and perhaps more trials of labor after cesarean delivery. 151 The proportion of women receiving blood transfusion was highest in lower middle income settings, where fewer preventive measures are available. 148 Although having registered for antenatal care is associated with a reduced risk of hysterectomy, unregistered women are likely overreported: it was often impossible to distinguish "truly" unregistered from referred patients who received antenatal care outside the center of surgery.…”

“…Under the National Health Insurance System, all Koreans are beneficiaries of medical coverage as either an employee or as a member of a community [9]. The Health Insurance Review and Assessment Service database contains information concerning the claims of ∽50 million Koreans [9] but, because it is a claims database, clinically relevant information such as smoking, alcohol use, body weight and height were not available.…”
