Trends in the Treatment of Dupuytren Disease in the United States Between 2007 and 2014

Abstract: Background: Dupuytren disease is a common fibroproliferative disorder. Multiple procedural treatment options are available, with Collagenase Clostridium Histolyticum (CCH) injection being introduced in 2010. The purpose of this study was to investigate trends in the treatment of Dupuytren disease in the United States between 2007 and 2014.

“…This database has been used in a variety of clinical studies. [10][11][12][13] The database provides an option for longitudinal tracking based on the date of a diagnosis, procedure, imaging event, lab result and/or a prescription medication. This function was useful for our particular analysis because it allowed us to track the amount of imaging paid for by Humana insurance after the diagnosis of AIS and/or LBP.…”

“…9 For more severe cases, 4 main treatment options are available, including fasciectomy, open fasciotomy, percutaneous needle aponeurotomy, and collagenase Clostridium histolyticum (CCH) (Xiaflex, Endo Pharmaceuticals, Inc, Malvern, PA) injection, with the goal of improving finger motion and function. 4,9 Fasciectomy has been the standard treatment for DC, involving surgical excision of the palmar fascia and, in some cases, fascia of the digit. Collagenase C histolyticum was approved in the US in 2010 to treat adult patients with DC with a palpable cord; it has become an established nonsurgical treatment alternative.…”

“…Currently, fasciectomy and CCH are the most commonly used treatments for DC in the US and therefore were the focus of the current study. 4 Since the introduction of CCH, use of both fasciectomy and fasciotomy for DC have decreased, whereas CCH injections have increased at a similar rate. 4 Clinical trials comparing CCH and fasciectomy are limited.…”
