Triple-Negative Breast Cancer in Ghanaian Women: The Korle Bu Teaching Hospital Experience

Abstract: Breast cancers that have negative or extremely low expression of estrogen receptor and progesterone receptor and non-amplification of human epidermal growth factor receptor-2 (HER2)/neu are termed triple-negative breast cancer (TNBC). The majority of TNBC tumors belong to the biologically aggressive basal subtype, and they cannot be managed with targeted endocrine or anti-HER2/neu agents. In western, high resource environments, risk factors for TNBC include younger age at diagnosis and hereditary susceptibilit… Show more

“…29-33 The contribution of germline genetic factors is supported by studies demonstrating increased frequency of TNBC among western, sub-Saharan women, a population likely to have shared ancestry with AA women as a consequence of the colonial-era trans-Atlantic slave trade. 29,34-38 …”

“…Nevertheless, the potential for survival gains may be hampered by the relatively young patient profile in SSA, reflecting the young age structure of the underlying population, as younger age is associated with poorer survival 13. Further, while a predominance of more aggressive breast cancer subtypes (eg, triple-negative tumours) in some settings would impede survival prospects,14 15 the totality of evidence across SSA from the largest studies and from a meta-analysis demonstrate a clear predominance of better prognosis oestrogen receptor-positive tumours 1617…”

“…Limited education, a fear of Western medicine and the observed adverse effects of breast cancer treatment including amputation of the breast and potential side effects such as post axillary dissection lymphedema lead to the preference of the majority of especially the rural population to seek care rst from spiritual healers. All of these factors continue to be known barriers to the introduction and adoption of evidence-based medicine in Ghana [13]. In fact, these also explain one of our study's major limitationsthe high attrition rate of approximately half the treated patient population.…”
