Triptan use and risk of cardiovascular events: a nested-case-control study from the French health system database

Abstract: The proportion of patients showing an overuse of triptans (more than 15 DDD for 30 days) reached 12% in this cohort of new users of triptans. However, we did not find any relationship between the overuse of triptans and cardiac outcomes. This study also shows that some patients with cardiovascular risk factors are actually treated by triptans. These patients are more likely to present a cardiac outcome potentially related to an ischemic event after the introduction of triptan.

“…[9][10][11][12][13][14][15] The fact that patients who filled their triptan prescriptions only once showed a significant decrease in the rates of physician office visits and diagnostic imaging after their index triptan fill is potentially suggestive of misdiagnosis. If these patients truly suffered from migraines, one would expect their medical services utilization rates to continue increasing on a trajectory similar to or worse than that of patients who frequently switched their triptans The fact that triptans seemed less effective at reducing the utilization of other services and medications among patient who switched triptans frequently is logical if triptan switching is viewed as an indicator of ineffectiveness.…”

“…9,11 The proportion of patients that fill only 1 triptan prescription without any further refills has ranged from 38% to 56%. 9,10,[12][13][14][15] Triptan brand switching is also a common occurrence. Roughly 15% of triptan users will try a new triptan over 1 year of follow-up.…”

“…Der Zusammenhang zwischen dem Einsatz von Triptane und kardialen Nebenwirkungen wurden in den letzten Jahren kontrovers diskutiert. Ob dabei ein Koronarspasmus die Beschwerden verursacht, ist unklar [5,6,7]. Das Risiko eines Myokardinfarkts bei einer Akutmedikation mit Sumatriptan ist sehr gering und liegt bei 1:1.000.000 [10] …”

“…This data is made available for academic research in the fields of medicine or pathology. The application of EHR systems is prevalent [1], including well-known systems as the Partners Healthcare Research Patient Data Registry (RPDR) in Boston [2] or the French health system database [3]. One major merit of this type of studies is that these databases collect cases with good demographic diversity.…”
