Introduction: Acne is a common disease of the pilosebaceous unit. Acne has been reported to be exacerbated by many factors, including psychoemotional stress. Aim: This study was conducted to assess the clinical severity of acne among acne patients seeking dermatological help in Ukraine during the stressful military conflict and to assess the influence of war on acne exacerbation. Material and methods: The study group consisted of 143 consecutive acne patients with the mean age of 21.24 ±6.37 years. The severity of acne was measured with the Investigator's Global Assessment (IGA) for acne. The selfreported acne exacerbation was studied with a single question "Have you observed exacerbation of acne during the war?". Results: Almost 45% of acne patients suffered from moderate to severe acne. The remaining ones had very mild and mild acne. The severity of acne did not differ between the sexes. More than half of patients (50.7%) reported that psychoemotional stress related to the ongoing military conflict exacerbated their acne lesions. No difference was noted in the flare of acne between female and males. Conclusions: Psychoemotional stress related to war activities exacerbates acne in more than half of patients. A prospective, multicentre study with the assessment of stress and acne flares by physicians will be of help confirming current results.

ImportanceWhile acne is common in transgender and gender-diverse people and is associated with gender-affirming hormone therapy, little research has examined these factors and their impact in gender minority groups.ObjectiveTo examine the lived experiences of acne and acne treatment in transgender and gender-diverse participants.Design, Setting, and ParticipantsThis mixed-methods analysis was conducted at a multidisciplinary gender center at a public safety-net hospital and endocrinology and dermatology clinics at a tertiary academic center from January 4, 2021, to April 7, 2022, using semistructured interviews and surveys. Participants were transgender or gender-diverse adults who had received gender-affirming hormone therapy. Data analysis was performed from November 11, 2021, to March 31, 2023.ExposureCurrent diagnosis of acne.Main Outcomes and MeasuresInterviews exploring the experience of acne and acne treatment were audio recorded, transcribed verbatim, coded using minority stress theory and the socioecological model, and analyzed using thematic analysis. Themes were triangulated with survey data on gender, self-reported acne severity, skin-specific quality-of-life impact, and treatment experience.ResultsA total of 32 participants were included in the study (mean [range] age, 32 [18-57] years; 17 transgender men, 11 transgender women, and 4 nonbinary participants). Ten participants (31%) self-rated their skin as currently clear or almost clear, 11 (34%) reported mild acne, and 11 (34%) had moderate to severe acne. Participants described experiences of rejection and bullying related to acne and admitted avoiding social interactions in which they anticipated acne-related discrimination, which led to feelings of depression and anxiety. Acne worsened body appearance dissatisfaction. Transgender women reported acne interfering with feminine gender expression. Transgender men often normalized acne development, sometimes viewing acne positively as an early sign of testosterone action. Most participants tried over-the-counter acne treatments and commonly sought acne treatment advice from physicians, peers, online forums, and social media. Barriers to acne treatments included cost, lack of multidisciplinary care, mistrust toward the health care system, and lack of transgender-specific acne care education.Conclusions and RelevanceIn this mixed-methods study, transgender and gender-diverse individuals reported experiencing acne-related stigma and facing barriers to acne treatment. Multilevel changes, such as developing strategies to reduce acne stigma, providing transgender-specific acne care education, facilitating multidisciplinary acne care, and expanding transgender-friendly clinical environments, are recommended to reduce the impact of acne in transgender individuals.

BackgroundThe disruption of the microbial community or dysbiosis alters the functional composition, metabolic activity, and local distribution of the microbiota leading the development of acne. The aim of this study is to evaluate the effect of a lotion containing a biotechnological phytocomplex, niacinamide, and succinic acid in the bacterial diversity of subjects with truncal mild‐moderate acne and its clinical benefits due to microbiota changes.Materials and MethodsOpen, clinical study in 43 subjects with truncal mild‐moderate acne treated with a lotion for 8 weeks. Bacterial diversity was analyzed by 16S rRNA gene sequencing of skin samples. Clinical effects were evaluated through IGA acne severity scale, biometric measurements, and safety.ResultsAfter 56 days of product's use, an increase in richness alpha diversity was found (p = 0.005), with a decrease in Cutibacterium acnes relative abundance (66.43% vs. 58.11%, p = 0.009). The clinical results showed a decrease in IGA score (27.59% decrease; p = 0.001), the inflammatory lesions (52.12% decrease, p = 0.006) and erythema (18.33% decrease, p = 0.007), and desquamation index (63.83% decrease, p = 0.02). The responder analysis of the IGA score showed that 60.47% of patients improved by at least one point at day 56. The product was well tolerated along the study.ConclusionThe use of the lotion on acneic skin was effective on rebalancing the microbiota, inhibiting biofilm formation and other virulence factors, reducing erythema and desquamation, and improving acne's severity.
