Tumor necrosis factor receptors (TNFRs) on T lymphocytes and soluble TNFRs in different clinical courses of sarcoidosis

Abstract: The evaluation of sTNFRs and TNFRs expression on T-helper cells may be useful in the estimation of sarcoidosis activity.

“…AMs from patients with sarcoidosis have been shown to produce both receptors in higher amounts than AMs from controls. 65,66 Most interestingly, although both TNFRs are upregulated by lipopolysaccharide (LPS) stimulation and correlate with the concentration of TNF, the release of CD120b disclosed the closest link to the TNF concentration. 66 The importance of these findings was shown by Ziegenhagen et al and Hino et al demonstrating in-creased levels of CD120a and CD120b in serum and BAL fluids.…”

“…TNFR1 (CD120a) is constitutively, but in low levels, expressed on nearly all nucleated cell types. TNFR2 (CD120b) is inducible and expressed by cells of myeloid lineage, peripheral T cells and alveolar lymphocytes and macrophages [18-23]. TNFR1 activates the caspase family, which induce cell death.…”

“…Approximately 25% of the peripheral blood lymphocytes in healthy subjects is CD4 + TNFR2 + . Interestingly, in sarcoidosis the percentage of CD4 + TNFR2 + cells is increased, especially in patients in remission or with stable disease (respectively 35% and 33%), which may indicate a role in down regulation of a cell-mediated immune response [23]. FoxP3 + T reg cells expressed the highest levels of TNFR2 among subsets of human peripheral blood CD4 + T cells and were able to shed large amounts of sTNFR2, suggesting an important role for T reg cells in suppressing TNF [25].…”

“…Possible serum markers including tumour necrosis factor receptor II have been proposed. 11 The Kveim test involves an intradermal injection of homogenised human sarcoid tissue extract. Although it has 50% sensitivity and close to 100% specificity, because of the impracticality of using human tissue the test is no longer used.…”
