Summary. The modal DNA values of 1465 tumours, together with other factors of possible prognostic importance, were related to the survival of the patients, using regression models (Kay, 1977). For most tumour sites except the testis, the distributions of modal DNA values were bimodal, with peaks at the diploid level and in the triploid-tetraploid range. For all tumour sites except the cervix uteri, patients in the low (near-diploid) range showed better survival; the reverse was true for squamouscell carcinoma of the cervix uteri. Other variables showed the following effects: for all sites except the testis, younger patients showed a better survival; for the cervix and corpus uteri, breast and ovary, increasing clinical stage was associated with poorer survival. Where evaluated, histological grade appeared to be associated with survival rate, the less well differentiated tumours having a worse prognosis, except for the breast, where the reverse correlation was noted. For carcinoma of the bladder, females had a poorer survival rate than males.

Twenty-five human breast cancers, surgically resected, were studied by cytogenetic analysis and DNA flow cytometry (FCM). The establishment of karyotypes showed that multiple cell populations probably were derived from a single ancestor clone, because common marker chromosomes always could be demonstrated. Differences of up to 30% were observed when the estimates of DNA content by the two methods were compared. A general tendency toward the acquisition of large marker chromosomes should be at the origin of this discordance, as the proportion Cytogenetic (CGj analysis has proved to be a powerful tool for studying chromosomal abnormalities in human malignancy (37,46). A number of primary events thus have been characterized, some of which are tumor-type specific. However, it is a long and painstaking method and even in expert hands, is successful only for a limited number of samples. DNA modal values first were estimated on interphase nuclei by static cytophotometry, which showed frequent deviations as compared with the content of normal diploid cells and which introduced the prognostic significance of this parameter (3,4,41). The development of several methods for obtaining DNA histograms from solid tumors by flow cytometry (FCM) has facilitated the study of a large number of samples (9,30,42,44). The application of DNA-FCM on archival material (18) has extended the possibilities by permitting retrospective studies with a long follow-up (10,17,19). However, results have shown considerable variability, often within a specific cancer, but also between the different tumor types. Furthermore, if aneuploidy has been generally recognized as indicative of a poor prognosis, it has been shown to have favorable incidence on outcome in childhood acute leukemia (26).Studies comparing CG and FCM results, on various tumor types have shown the existence of discordant cases on the basis of DNA content (2,32,43). We have of markers for each case correlated significantly with the magnitude of the difference. Parallel use of the two methods revealed the existence of tumors with DNA diploid FCM profiles and highly abnormal hypodiploid karyotypes (35-40 chromosomes), which may explain the limited value of DNA ploidy as an independent prognostic factor in breast cancer.Key terms: Human breast cancer, DNA ploidy, cytogenetics, prognostic value shown previously that quantitative measurements of DNA (chromosome counts and DNA indices) are highly correlated in colorectal cancers (34).We are reporting here our CG and FCM results for 25 cases of human breast cancer. Gleason-score <=6, assessed on core needle biopsies, is an essential prognostic parameter to offer the strategy of Active Surveillance (AS) to patients with locally confined cancers of the prostate. Yet, its interobserver reproducibility is low (48-70%) and its prognostic validity unsatisfactory. An option to complementary assess the malignant potential of these cancers are objective DNA-ploidy-measurements on existing biopsies. For that purpose chromosomal heterogeneity is indirectly quantified by DNA-cytometry resulting in DNA-grades of malignancy 1-4. This review systematically trawls and evaluates all scientific publications on the potential diagnostic and prognostic validity and the heterogeneity of DNA-ploidy measurements in cancers of the prostate between 1966 and 2013. Publications have been classified into Oxford levels of evidence and levels of significance were given for the correlation of DNA-ploidy with different clinical outcomes. 114 scientific articles had to be excluded because of different methodological reasons. All but one of the 67 methodologically acceptable articles report on a significant diagnostic resp. prognostic significance of DNA ploidy measurements in cancers of the prostate. 8 level 1b studies report that DNA-ploidy, assessed on punch biopsies independently predicts organ confinement as assessed after radical prostatectomy. 18 level 2b studies prove that DNA-ploidy measurements add statistically significant information to the Gleason-score. 16 level 2b investigations report a significant correlation of DNA-ploidy with recurrence-free survival. 15 level 2b studies document a significant correlation with overall survival after different types of therapy. 5 level 2b investigations prove a significant correlation with local recurrence or progress after radical prostatectomy. 3 level 2b publications show a significant correlation of DNAploidy with the occurrence of lymph node-or bone metastases after radical prostatectomy. 1 level 2b study documents the additional prognostic value of DNA-ploidy measurements over conventional subjective grading in prostate cancer patients under AS. All existing 15 narrative reviews on selected articles dealing with prognostic DNA-cytometry in cancers of the prostate are in favor of this method. Prospective level 1b studies, especially those proving the validity of DNA ploidy measurements to predict non-progression in patients with clinically insignificant low-grade low stage cancers of the prostate eligible for Active Surveillance additionally to the Gleason-score are still missing.
