A publication was excluded when the team itself was the innovation (i.e., ''Introduction of twenty-four hour care teams''; Townsend, 1994) or the team itself was the implementation strategy, i.e., ''The role of a specialist team in implementing continuing health care guidelines in hospitalized patients'' (Cockram et al, 1997). Data published in duplicate were included only once.…”
