Two cases of acute methanol poisoning partially treated by oral 4-methylpyrazole

Abstract: While the use of 4-MP is better documented in cases of ethylene glycol poisoning, it could also become an accepted option for the management of methanol poisoning since 4-MP offers advantages over ethanol therapy.

“…Prevention of both ocular toxicity and accumulation of formate are accomplished by administration of fomepizole in intoxicated monkeys [14]. Only a small number of human methanol poisonings have been treated with fomepizole [15,16,17,18,19,20,21]. At least nine of these patients underwent hemodialysis [16,18,19,20] and three had associated ethanol therapy [15,16].…”

“…The study included 14 patients: nine men, five women; median age 46 years, range (Table 1). Two cases (patients 2 and 8) have been previously reported [16]. The ingested products were cooking alcohol (n = 7), pure methanol (n = 4), windshield washing fluid (25 % methanol, 9 % isopropanol; n = 1), or undetermined (n = 2).…”

“…12,13 Although this was our preferable method of treatment it was not possible for us to find the drug immediately therefore we start the treatment using ethyl alcohol which competes with methyl alcohol for the alcohol metabolizing system. Administration of ethanol for the treatment of methanol intoxication is therefore common once alcohol dehydrogenase has a much higher affinity to ethanol than to methanol.…”

“…The major objective of our case was the delayed toxicity of methanol in combined ethanol-methanol poisoning. Tachycardia can be absent in some cases of methanol poisoning, as in one of the two cases reported by Hantson and colleagues [3].…”
