Two cases of adenodermatofibroma

Abstract: Dermatofibromas (DF) are common skin lesions composed of a dermal proliferation of fibroblasts and histiocytes. Among the variants of DFs, adenodermatofibroma are characterized by a dense proliferation of fibroblasts and histiocytes admixed with entrapped dilated glandular structures. We report two additional cases of adenodermatofibromas, review the literature, theorize on the histopathogenesis of this variant, and suggest that there are different patterns among adenodermatofibromas, from primarily cystic to … Show more

“…In cases with a single cystic cavity, etiology is likely secondary to blockage of a single eccrine duct, leading to dilation and subsequent apocrine metaplasia. 5,6 In cases with several small apocrine glands, there is no blockage, but adjacent eccrine glands undergo induction and apocrine metaplasia. Muto et al 5 propose that this process of apocrine metaplasia is analogous to processes that lead to cystically dilated glands in adenolipomas and apocrine hidrocystomas.…”

“…5 For example, a case identified by Santos-Briz demonstrated multiple glandular structures evenly distributed throughout the lesion with focal cystic dilation; in our case, only one central dilated cyst was identified. Leiphart et al 6 theorize that these variations are because of slight differences in pathogenesis. In cases with a single cystic cavity, etiology is likely secondary to blockage of a single eccrine duct, leading to dilation and subsequent apocrine metaplasia.…”
