Ulnar Neuropathy Around the Mid-Arm Combined with Martin-Gruber Anastomosis

Abstract: This study reports a rare case of ulnar neuropathy around the arm with Martin-Gruber anastomosis of a moderate conduction block in the forearm segment and a severe conduction block in the arm segment. Inching tests and ultrasonography showed a lesion between 12 and 14 cm from the medial epicondyle. It is concluded that axilla stimulation may provide diagnostic clues, and inching tests and ultrasonography may be helpful for localizing a lesion.

“…The opposite happens with UN stimulation, as the elbow response is smaller because the UN receives crossing fibers in the forearm, resulting in a bigger response at the wrist [ 6 , 17 ]. Hence, MUC might be rarely misinterpreted as a conduction block of the UN between the wrist and below-elbow sites [ 17 , 18 , 19 , 20 ]. Of interest, MUC may lead to interpreting errors during routine nerve conduction studies (NCS) in patients with carpal tunnel syndrome (CTS) [ 5 , 11 , 12 , 21 , 22 ].…”
