Ultrasonographic Confirmation of Dorsal Ulnar Cutaneous Nerve Neuroma Following Hand Surgery - A Case Report -

Abstract

-Pseudomeningocele after spine surgery can cause various symptoms, but it can also be silent. We experienced 3 cases of pseudomeningocele with different symptoms and we analyzed the characteristics of each case. A small pseudomeningocele without connection to the subarachnoidal space can show no symptoms. A pseudomeningocele with a small dural tear and it's abutted on the duramater at a small portion can produce sciatica and limitations of straight leg raising due to adhesion of the cauda equina around the dural tear. In addition, a large pseudomeningocele with a big dural and lamina defect can produce back tenderness furthermore, a patient with such a lesion can have low back pain and leg pain that are aggravated by an increment of abdominal pressure or by impact to the body and even by walking. Pseudomeningocele should be suspected when symptoms recur after spine surgery and especially in the case of dural tear during an operation Key W Words: pseudomeningocele, spine surgery 서 론 척추 수술 도중 경막의 의인성 손상으로 뇌척수액의 누수가 발생한 경우는 이를 봉합해 주어야 하며, 수술 후 뇌척수액 누공이 발생하면 다시 재개방하여 봉합을 하여야 하는 것으로 알려져 있다 1,2,34). 그러나 대부분의 가성수막류는 무증상이어서 누공을 형성하지 않는 한 발견되지 않는 것이 보통이다 1,2,3,5). 또한 증상을 유발할 경우, 이에 대한 특징이 매우 드물게 보고 되어 수술 후 수개월 이상 경과된 후에 발생한 일련의 증상이 가성수 막류가 원인인 것으로 고려하기가 쉽지 않다 1,2,3). 미발견 된 가성수막류는 수개월이 경과된 후에도 증상을 유발 할 수 있으며 침습적 통증 조절 치료 등을 통해 의인성 으로 누공을 형성하거나 감염의 위험이 있으므로 수술 후 뒤늦게 발견될 수 있는 합병증으로 항시 염두에 두어 야 할 것으로 생각된다. 저자들은 요추 수술 후 6개월 이 상 경과된 후 발견된 가성수막류를 3례 경험하여 각 증 례에 따른 임상 증상의 특징을 보고 하고자 한다.
