Ultrasonographic Screening for Gallstone Disease in Middle-Aged Women

Abstract: Five hundred and forty-seven middle-aged women, selected at random from the population of Malmö, Sweden, were invited to a screening survey for gallstone disease; 424 participated (77.5%). Forty-one had previously been operated on for gallbladder disease. The prevalence of gallstone disease, on the basis of a positive finding at ultrasonography and cholecystography, was 11%. The predictive value of a positive finding at ultrasonography was 86%. Six out of 10 women with gallstone were classified as asymptomatic… Show more

“…In accord with others 15,28 , the present authors did not find increased plasma triglyceride concentrations among subjects with gallbladder stones in their prevalence study 17 . In the present logistic regression analysis using baseline measurements as independent variables, plasma triglyceride levels were not related to gallstone development.…”

“…Older age and female sex have been shown to be strong risk factors for gallstone disease 1,9,10,17,22 -24 . Several 7,23 -27 , although not all 28,29 , studies have shown a positive relationship with high BMI. In the prevalence study of the present cohort, BMI did not differ between subjects with and those without gallbladder stones, although individuals who had previously undergone cholecystectomy had a significantly higher BMI 17 .…”

“…2 In part, these contradictory findings are related to differences in research design. Some studies are uncontrolled, [13][14][15] other reports present findings from patients seeking medical care, [16][17][18] from case-control studies, 15,[19][20][21] or from community-based studies on selected [22][23][24][25][26][27] or unselected [28][29][30][31] populations. A recent meta-analysis, which takes into account all the studies issued on this topic from 1983 to 1992, concludes that only upper abdominal pain (with no preference for the right side)…”

“…64 Increased prevalence in females, especially in those with high fecundity, was observed primarily in European and American populations. 19,21,56 Explanations for the differences in the prevalence between these populations may be obtained by comprehensive sonographic studies of large populations that document factors such as subjects' nutritional habits and familial disposition for cholecystolithiasis.…”
