Ultrasonography as an aid to diagnosis and treatment in a rural African hospital: a prospective study of 1,119 cases.

Abstract: Abstract. The aim of this study was to assess the utility of ultrasonography in a rural African hospital in Cameroon with scarce resources. A prospective questionnaire was administered and completed for each of the 1,119 consecutive cases included in the study. Among these 1,119 cases, the diagnosis made by clinicians and by echography could be verified by another means for 323 patients. Ultrasonography showed abnormal findings in 78% of the cases. In the group of 323 patients in which the diagnosis made by ec… Show more

“…The role of ultrasound must be weighed against the available treatment options and the patient's willingness and financial resources to proceed with treatment. 11 In some cases, the diagnosis of possible cancer influenced the course of treatment in a completely different direction; unfortunately, a late diagnosis of malignancy increases the likelihood of metastasis, and in a country with limited treatment options, the impact on mortality was unclear; however, a more accurate diagnosis is obtained. In other situations, for example, the child with a draining sinus tract and evidence of osteomyelitis, the patient could be expeditiously transferred to an orthopedic hospital for an attempt at salvaging the limb.…”

“…An early study of the utility of ultrasound as an adjunct to physical examination was carried out from 1991 to 1993 in a rural hospital in western Cameroon with a donated ultrasound device. In 323 cases where a gold standard was available, ultrasound provided the diagnosis in 31.6 % of the cases, confirmed a prior diagnosis or excluded alternate diagnoses in 36.2 % of the cases, had no influence on the diagnosis in 27.6 % of cases, and was incorrect in 4.6 % of the cases [55]. Another study reported experience introducing portable ultrasound into the health services of a refugee camp in rural Tanzania [56].…”
